In order to certify an illness, please request a sick cert at the time you are sick.

Submitting the form does not guarantee a sick cert will be issued. We may need to arrange a phone consultation or an in-person consultation with your doctor to certify your illness (see fees here). In this instance, the cost of the certification will be included in the charge for the phone/in-person consultation.

The normal cost of certification is €20. This fee includes both a sick-note for your employer and a digital submission to the Department of Social Welfare. This is the fee for all private patients. Our reception team will send a digital link to your phone to pay the fee.

If you are a Medical Card or Doctor Visit Card patient, and you only require a Social Welfare Cert, there is no charge. However, there will be a charge of €20 for a sick-note for your employer.
