Click below to register your child for an Under 8 GP Visit Card. You'll need an email address, a parent's PPS number and your child's PPS number in order to register. Click on one of the doctors from our clinic to apply. If your child is under 2 months of age and does not yet have a PPS number, leave that section blank and the HSE will contact you in due course.
If you are over 70, you can apply for a GP Visit Card, regardless of income.
Medical Card / Doctor Visit Card (DVC)
Applications for GP Visit Cards and Medical Cards are made through the same system. You will first be assessed for a medical card. If you don't qualify, your will then be assessed for a GP Visit Card. You will need your email address, date of birth and PPS number.

Under the Long-Term Illness (LTI) Scheme you can get some drugs, medicines and approved appliances for free from your pharmacy. There is no means test for the scheme. Illnesses include diabetes, epilepsy, Parkinson's Disease and others. Click on "Apply" at the top of the page to access the application form.
If you have been "substantially restricted" by an injury, disease or physical / mental disability and are aged between 16-66, you may be eligible for Disability Allowance. You need to have been / expected to be disabled for at least a year. You need to complete your section of the form before bringing it to your GP.
Carer's Allowance is a payment to people on low incomes who are looking after a person who needs support because of age, disability or illness (including mental illness).
Please complete your portion of the form if you need to attend for a driver's medical examination due to your age, medical conditions or if you drive a group 2 vehicle. If you wear glasses, please bring them with you. Please note that this service is not covered by the medical card and a fee will apply.
